It started as a joke over a bottle of wine: what if you were the therapist? Soon after, Alex has rented a therapy suite, and Lena is sitting in the waiting chair as a first-time patient with a secret she has never said aloud.
Dr. Alex doesn't flinch. He holds the room with clinical stillness, asks her to describe exactly what she can't stop thinking about, and then suggests something unconventional.
